Many of CareCognitics customers were already facing challenges from Flu season when the COVID-19 pandemic hit. Digitally Screen Patients

Using the CDC guidelines, our chat bot will screen patients to identify high-risk or symptomatic patients who should be tested.  Patients using smart phones can remain digitally connected to your organization from the screening process all the way through testing – lowering the risk to your staff and your patients.

Educate Asymptomatic Patients

Patients who are identified as asymptomatic or low-risk are educated on social and physical distancing practices as outlined by the World Health Organization and the Centers for Disease Control.

Send Patients for Testing

At-risk patients and symptomatic patients are provided information on testing locations; their information is collected digitally for quick processing at the testing site.

Digital Results to Patient

The option to provide testing results digitally is available for cases where there is no need for interaction with a provider or a provider is unable to reach the patient by phone.

Real Time Insights and Trends

CareCognitics Insights provides aggregate data on the number of screenings, results of screenings, symptomatic cases by city and zip code, tests completed, and test results. Use these insights to plan for additional testing locations, identify areas of outbreak, and understand community health needs.

CITYWIDE COVID-19 SOLUTION WORKFLOW


HOW DOES IT WORK?

+ Healthcare providers know that patient outreach has to be simple, so the process starts with the patient texting “covid” to a dedicated number.  They immediately receive a link to a COVID-19 screener. Patients walk through a straightforward text-based screening tool that performs risk stratification, directing those patients who need testing most to a centralized drive-through testing station.  As the COVID-19 pandemic evolves, screening questions can be quickly adapted to match the most current guidelines with minimal client effort.  

+ The screener gathers demographics including name, date of birth, and address – this allows automated registration prior to a screening appointment.  Public health experts can use a COVID-19 dashboard to monitor the number of patients accessing the screener along with a breakdown of symptomatic vs. symptom-free patients.  Symptomatic cases are further stratified by zip code.

+ Once patients arrive at the screening station, their information drives real-time dashboard updates including laboratory order status.  Once results are received, positive and negative results can be mapped back to zip codes, allowing for targeted public health intervention.  Patients can be notified of their results through personalized MyCarePlan content which provides follow up instructions and patient education.

+ The heart of this rapid implementation includes state-of-the-art interoperability tools based on the FHIR (Fast Healthcare Interoperability Resource) Specification.  This allows for rapid modification of content as well as real-time data visualization which can drive clinical interventions.

+ During the first week using the solution, over 8000 people were digitally reached with 4500 patients completing the COVID screening. More than 90 percent used mobile phones. Over 1500 COVID tests were ordered. Automated scheduling and laboratory order generation created efficiencies for the drive-through testing center, which was able to increase its throughput by more than 300 percent.
